International Letter-post Items Regulations (SOR/83-807)

SCHEDULE III(Paragraph 7.1(3)(a))

The CP72 form (Customs Declaration Dispatch Note) must be completed by the sender and securely affixed to the item (on the address side when possible), when the value of the letter-post item exceeds $500. The form contains information required by Customs authorities in other countries. This information includes the name and address of the sender and recipient, a detailed description of the contents and the sender’s signature.
